Q: What is the warranty? Has anyone used the warranty past the Amazon 30 day coverage?

I have had them on my 2006 gmc 2500 HD for 4 years now. Still just as solid as the day I bought them. These are great shocks. Especially if you tow heavy weight

Q: will they fit on a 2008 2500 hd duramax with just 2 little leveling kit blocks in it ?

I have them on my 2000 gmc sierra 2500 with the torsion bars cranked and they work great. Depends how big your blocks are I guess.

Q: Will the 5100 series work well on heavy duty application. i mainly tow a 12000 lb fifth wheel and not sure these will be like the ones as stock

In 2014 I put them on my 2007.5 chevy 1-ton diesel and I tow a 14,000 lb fifth wheel. I had also installed Sulastic shackles and GM's hydraulic rear cab mounts in 2011, so it is hard to say how much improvement you would see. The shocks have been on for about 10K miles, and show no signs of leakage.
