Q: Will this fit on a 4" grinder?

I would not recommend putting this buff on a 4" grinder.

Q: Can i use w/ a standard 4.5in grinder? I would use extension & safety flange

You can, however the larger buffing wheels work the grinder really hard. You are more likely to burn up your grinder if you are doing heavy cutting.

Q: Can you mount these on a bench grinder and operate safely?

Maybe, the wheels have different recommendations for rpm’s, I don’t think most bench grinders have variable speeds
