The tank is never to have any water past the bladder inside the tank. That area is reserved for you to fill it with air to match the static domestic water pressure. That's the pressure you have when water is stopped at the pressure gauge and not running any place else. So the tank is 50% air and 50% water. If the tank is full of water it is in failure. If you are talking about the thread protector cap yes you have to remove that, but don't damage that rubber plug/bladder inside the tank. And if you are over your head, call a licensed professional. If water is coming out, then the interior bladder is busted and you need a new expansion tank. They are inexpensive and easy to install.

Q: Will this work for a 75 gallon gas hot water tank, brand is AO Smith?

According to sizing charts the Amtrol ST-5 is too small for a 75 gallon hot water system. I am using the ST-5 but only for a 40 gallon system.

Q: Does this have to be mounted from the top or can it be turned over and mounted?

Instructions state to mount only in a vertical downward position. Do not mount facing up. Separately, I also recommend a metal strap if possible. If you live in an earthquake zone, this could easily sway, causing a leak in your pipe that could flood your home.
