Exactly as pictured.
I’m using these to strengthen my pelvic floor. I’m currently wearing the big one for an hour a day. Next week I’ll wear it for two hours up until I wear it for five hours. After a few months I’ll move to medium size one doing the same procedure. I tied dental floss through the loop on the end for easy removal. Wash well after every use and allow to dry before using again the next day. I recommend these for this purpose.

I ordered this for health reasons, but they were of no use. Also, I became concerned about contamination. I personally would urge caution about using such products. Maybe you could use the big one on a wind chime. lol 😄
