Manually. The initial setup locks the wheels in the direction you decide on.

Is there a medium size of this rolling tray?

Haven’t seen any scratches and I have one for coffee pot also.

Does it scratch counters?

No, it doesn't scratch. Without it, it was hard to slide. Now it's easily glides. Glad I bought it.
