Q: Is it designed for outdoor use ?

It is a well painted steel. I think you could use it outdoors as long as you maintain the paint.

Q: Can this be used in cinder block walls?

Not with the bolts & sleeves they provide - not enough thickness on the concrete for the sleeve teeth to grip. I bought some new toggle anchors & bolts, it is solid as a rock attached to the block wall.

Q: The mounting holes, left to right need to be 32" so that it can be fastened to the studs. What the center to center distance of the mounting holes?

on the one with the full plate the edge holes are 19 3/4 center to center

Q: Do you recommend a specific paint or spray to prevent rust when mounted outside?

The surface of the bar has been treated with metal coating to prevent rust. As long as the surface is not scratched, it will not rust.
