Nice book
Returned because not what I needed, but good item for documentation

Good little record book
I am the "secretary" for all things medical for myself, my hubby, and my mom, so I have to keep decent records to keep things straight. For several years I've just used a spiral notebook and tried to organize it to accommodate the info I need to keep track of. This little book just makes it much easier, with a place for all the different kinds of information I need, from doctor's names, phone, address, lists of immunizations, a place to record procedures done, plenty of room to keep track of medications, and then a two-page spread for collecting info from each office visit. It's also smaller than the spiral I was using and easier to tuck into a bag or purse. It even has an elastic band bookmark so you can mark your place for the next office visit. The only thing I'd change is to put a loop for holding a pen (because I never have one when I need it!) and maybe a pocket in the back for holding the appointment cards or important printouts the Dr. gives you. It's not perfect, but it's a pretty useful little book that does what I need it to.

Pretty Good for Us, Could Use A Few More Sections
This journal works pretty well for tracking my daughter’s complicated illnesses. Lots of room for medications and doctor appointments, including what tests were ordered and the results. I wish I had it earlier to track what medications she’s tried and why various ones didn’t work. Everything suddenly became more than I can remember. I will probably buy a book for my healthy daughter and for myself, and staple or glue in pages for imaging. I wish it had a section to list imaging and concussions, a longer section for ER visits and specialists, and a separate section for surgeries and overnight hospital stays. Tabs would be nice as well. Nonetheless, it is far better than the system I was using as I was getting completely overwhelmed by the information I had to keep track of for the various doctor visits. I will probably end up making my own book in a small 3-ring binder to customize it for her specific needs so that I can continue to add to it. This book will only last about a year or so for her. Probably a lifetime for most people.

Clear, concise and organized.
I have a chronic health issue and a spouse who travels frequently. After having a recent medical emergency, it became apparent that I need to keep my records all in one place. I haven't filled it out yet, but it is very thorough, and has space for multiple specialists, my latest appointments and medications. It is a book that everyone should have filled out, and is especially helpful for those who are technology impaired, distrust full of computers or simply desire a hard copy as back up. I recomentld this for spouses, caregivers, and to stash in a bag while traveling.- J. Chambers

Just what I was looking for
I needed something to keep my parents' medical information for my own information, especially since when my mother was hospitalized earlier in the month I could not tell the ER staff more than 2 of her medications and no details about those either. I bought two of these books, one for each parent, and filled them out with them. There is a page for insurance information, space to record 30 different medications, hospitalization/surgery/ER visit record, pages to list all doctors and specialists seen, assistive device information, eye and dental records, and then two large sections - one called Ailments History that I use when one of them calls me to tell me about a problem they are having (3 per page), and one to record physician visits (two pages per visit). These pages have a place to record the date of visit, doctor's name and phone number, reason for visit, diagnosis, any tests ordered and results, vitals, prescribed treatments, new medications, and a half page for general notes. Since I now am driving them to their visits, this is really handy for me to have, especially since both have impaired memory.These are designed to be your own personal medical record, but I highly recommend it to anyone caring for an aging parent.

Not quite there
This book is a good idea except it needs more organization. Tabs with dividers maybe? If you don’t go to the doctor that often it should be fine. But those of us who go once or twice every year to more than just our general practitioner, then there needs to be more divided sections.
