What is the actual case diameter? I've seen 41.2 mm, 43 mm, 38 mm ... the specs seem to be all over the place. Does anyone know for sure?

There isn't. You'll have to purchase an oversized protector and cut it to size yourself.

Does this watch keep the correct day/date year over year?

Yes, I just checked with my husband and it has today's date. He has had this watch for a long time and even takes it outside when he is working in the yard. It is great for the priced! Hope this helps.
