Q: Is it best to use it on it's own or with an ordinary pillow behind it?

It's personal choice. I tried different ways to use this batwing pillow after shoulder surgery. Reason I chose this was due to the central back support which is very good. I used with another pillow behind.

Yes,I just lie it flat on my mattress and it still gives semi upright position,if you need it higher just put normal pillow under it to suit
