I would say it is accomodating for larger heads. Bought it for my husband who is a full size 6’1” & weighs 230. It fit him fine. Very nice & warm since it is lined.

Why didn’t I receive the hat when it said it was delivered

That I can't truly answer. It could have been a few different reasons as to why it was't delivered. Someone could have stolen your package or it could have been delivered to the wrong location. Also if you live in an apt building the front office could have possible held it for you to pickup. I would reach out to Amazon and advise them what happened and they can do an investigation to see what really happened .

Does this hat work to cover my ears for large heads?

Tight for my average size. May be uncomfortable.

How do i let someone know that i did not receive the item, but an empty envelope?

That DOES happen. I ordered something and was delivered an empty envelope. Fortunately I was there at the time and spoke to the Amazon delivery person and called amazon right away, photographed the envelope and delivery person. I had him call his Supervisor as well. So yes, it happens!!!
