Does it Have two inlets for the pneumatic tubes from the cistern?

The button has two inlets . Please note that I used this on a one tube cistern. Hope this is helpful.

Hi does it come with the mechanism behind the button that you attache/insert the tube into?

No it doesn't.It comes with the small tube and you just connect that to the flushing mechanism but it doesn't include the whole flushing mechanism.Very easy to fit once you've figured out how to remove the old one

can someone thelp me with answer. i have to push and hold the button in to get a full flush is that correct.

In reply.. it depends on model of toilet, i.e. wall hanging and size/make of cistern such as Grohe Rapid Xl, Geberit or VIDA XL .. On my Grohe XL wall hung cistern, I press both 'flush plates' for short time and then release the smaller flush which will give you maximum flow. Normally a single long push is sufficient to give you the required (large / small) flow. Not sure if the water pressure into cistern has anything to do with your query, i.e. is cistern full when pressing plate ? Good luck!

Please confirm this is suitable for a horizontal surface rather than a wall?

I've never seen a Grohe cistern with a top surface option. All appear to be face fix only.
