Its meant to pick up sound only from front of mic and a little to the sides. But let me tell you, I have this mic and it picks up sound from all around, not what I wanted at all.

Would this be suitable for voice to text? I have arthritis and am struggling to use a keyboard.

Yes, it would. I had a shoulder injury and couldn't do the necessary paperwork for my job. I invested in Dragon NaturallySpeaking voice recognition software but didn't like the headset that came with it. I bought this after researching them. This is a fantastic microphone.

Does this have the 3 modes at the back of the microphone?

Negative, just a single Cardiod mode (says its single direction 180 but picks up everything 360).
