Q: Has anyone applied it to the inside of a case?

You need to put it on the outside of your case. It's very thin, so it may physically fit inside, but the strength of a magnet decreases by the distance between the objects squared (inverse square law). Even a few mm of plastic between the adapter and the magnet would significantly reduce the attachment strength. This adapter is very thin and shouldn't be too intrusive, but it does need to be on the outside (I have tried placing inside the thinnest case Spigen makes for the z fold 3. It doesn't work well through the case). I also recommend adding your own adhesive since I have had issues with the adapter peeling in the past. Super glue fixed that problem.

Don’t listen to that other person, they don’t know what they’re talking about. Yes it does work. I have it applied to the outside of an Otterbox case and it works 100% as it should.

It's a metal ring you use with Magsafe products.

Q: Will it fit the samsung z flip 3?

I think that the ring will be too large because of the little tail piece that comes down from the bottom of the ring. A true circle ring would work if you can adhere it to the bottom piece of a smooth-backed case. The bottom part of the Flip is where the charging coils are.
