I have never used this before but do I need to install the Heat sink or is it already pre-installed so I place it on my ps5?

According to the SSD installation guide of  SONY PlayStation 5,  TEAMGROUP T-FORCE CARDEA A440 1TB/ 2TB are meet the requirement with the interface, capacity, sequential read speed, and so on.

Please note that this product comes with two heat sinks. You may need to install it by yourself. 
One is a Graphene heat sink (with 3.7mm height) that could be fit in the PS5 SSD slot. Another one is an Aluminum heat sink (with 12.9mm height) that could install in the PS5 SSD slot without the slot's lid.

Hello will the 4tb sad work with ps5 and does it fit perfectly with the provided heat sink. or will i have to get a different heat sink???

4TB is the max usable with a PS5. I used the included heatsink with the Admireable model and it fits in the PS5 drive bay with no problems.

What size screw do I need for this?

The screw size is 2.0 x 3mm (CM2x3-3.3).

Please check you motherboard's screws, it should have some extras if not, you may just take one directly from your motherboard as a sample and you can buy in any hardware store.
