Is this hat real or fake?

These hats are purchased directly from the licensed Ariat Accessories manufacturer

Does it run small or large?

I wear a 6, 7/8 size, this hat happens to be right in between the last snap and 1 past that for me. I would say it runs slightly big but probably not for someone with a much larger size than mine.
