How well does this work on small citrus such as key limes?

I haven't tried key limes. At BJ's (the local equivalent of Costco, etc.), they sell bags of undersized limes which aren't much bigger than key limes, and it works fine on them. You press down on the fruit with the palm of your hand on to a spinning plastic apparatus. It isn't dangerous, and I think you'd be fine if you're careful.

Does it take a lot of strength to juice oranges with this juicer? I have arthritis.

No, I also have arthritis in my hands. I love this juicer,,,am going to make another 33 jars of lemon curd in Feb (we spend the winters in Arizona) and will have to juice over 100 lemons. This is a great product,,I love it!!!!!

How loud is it? There are several different responses. Can you comfortably hold a conversation while it is operating?

When it works, it is quiet and fast and you can hold a conversation fine while using it. After three months mine started making a loud grinding sound and the turbo function stopped all together. It sounded like the gears were no longer aligned.

Is the reamer stainless steel or plastic?

It's plastic, and it is integrated with the pulp strainer.
