Q: Will toilet paper fit?

I’m sure it would...but it is the length of a roll of paper towels ....so toilet paper would probably move back and forth on the dowel

Q: Is the back supposed to screw on

There is a plastic blue cap over the screw that sticks out of the end of the wooden dowel that goes into the butt. I left that plastic cap on so when I change the roll of paper towel the uncovered screw cannot scratch my counter top if it hits it. It also makes for a tighter fit. Changing the roll of paper towels is easier sliding the butt of the plastic cap then unscrewing the butt every time.

Q: Are the head and tail made of metal?

Cold cast ceramic is made with resin. I can't stand cheap resin garbage

Q: Can this be mounted on the wall?

No, it is made to stand on coumter
