Exactly what size cap end wrench is required for these M1-110 filter

I have an expandable strap wrench, as well as another chain one, but the distance from one side of the filter end to the other is 2 and 1/2 inches. I hope this helps.

I'm using high mileage regular oil in a 150,000 mile engine.when i change oil at 5,000 miles, do i need to change this oil filter at every oil change?

High mileage oil is only for cars that are currently leaking oil as the high mileage oil contains seal swellers. If you are using high mileage oil on an engine that is not currently leaking oil, the seal sweller additive will most likely cause the seals to start leaking. & yes change your synthetic oil filter when oil is changed.
