Q: Why is there 3 holes in the top of the 1" die plate,assuming for wider boards but not sure.

It has to do with the width of the joists. 1-2x or 2-2x or various width. It allow the the user to tweak the amount of leverage or force to place on the deck boards. It really does not have any relation to the width of the decking boards. Sometimes it could be use for adjusting for the lack of space to place the tool.

Q: Will this work with 4x6 joists (about 3 5/8 inch wide)?

It will not adjust that wide. It has two settings for the pins. One is for a standard 2x's (1.5") and I believe the other is for sistered 2x's (3"). You may have to make a custom prybar to move 4X6 posts.

Q: Why it is so expensive as compare to camo level ?

I don't know about the camo tool, but as a professional builder, I highly recommend this tool. A week after buying the first one, I bought a second one. It's a real solid, quality tool.

Q: Will this work just as well if you lay the top boards at a 45 degree angle to the joists?

You should be able to arrange the tool in a way that you can still squeeze the decking into place when placed at a 45° angle to the deck joists. However, when choosing your decking material, it will be important to consider whether it is rated to span the additional space created by placing it at a 45° angle across the floor joists.
